01261: PILOT PUBLICLY FUNDED CHILD CARE MANUAL
CLAIM/ADJUSTMENT FOR ATTENDANCE
This form can be downloaded and copied from the ODJFS Child
Care site....THIS FORM MUST BE USED FOR ABSENTEE DAYS
AND ANY REASON YOU MIGHT HAVE TO MANUALLY REPORT
ATTENDANCE....Please print and keep on file for future use.
